What Should My Lipoprotein(a) Level Be?
Introduction
If you have recently had a blood test that included lipoprotein(a) — usually written as Lp(a) and pronounced "L-P-little-a" — you may be looking at a number on a laboratory report and wondering what it actually means. Perhaps your result was flagged as raised, or perhaps a relative has been found to have a high level and you have been advised to get tested yourself.
Lipoprotein(a) is one of the most important yet least talked-about markers in cardiovascular medicine. Around one in five people carries a raised level, most without knowing it, and unlike ordinary cholesterol it is determined almost entirely by your genes rather than your diet or lifestyle. A high Lp(a) level is independently associated with an increased risk of heart attack, stroke and aortic valve disease — which is why international guidelines now recommend that every adult should have it measured at least once in their lifetime.

What Is Lipoprotein(a)?
Lipoprotein(a) is a particle in the blood that carries cholesterol, similar in structure to LDL ("bad") cholesterol but with an additional protein attached, called apolipoprotein(a). That extra protein changes the particle's behaviour in ways that matter for the heart and blood vessels:
- It promotes the build-up of cholesterol in artery walls (atherosclerosis), in the same way LDL does.
- Its structure resembles a natural clot-dissolving protein called plasminogen, and it may interfere with the body's ability to break down small blood clots.
- It appears to promote inflammation and calcification, which is why high levels are linked to aortic valve stenosis — a progressive narrowing of the heart's main outflow valve.
The crucial difference from ordinary cholesterol is that your Lp(a) level is 80–90% genetically determined. It is set largely by the LPA gene you inherit, is established in early adulthood, and remains broadly stable throughout life. Diet, exercise and weight loss — so effective for LDL cholesterol and triglycerides — have very little effect on Lp(a). This is not a reason for fatalism; it simply means the response to a high level is different, as explained below.
Because it is genetic, a raised Lp(a) runs in families. If one of your parents has a high level, there is roughly a 50% chance you have inherited it too — which is why testing is often extended to close relatives when a high level is found ("cascade testing").
What Should My Lipoprotein(a) Level Be? The Numbers Explained
Here is the answer most readers are looking for, with an important caveat about units first.
Check your units: nmol/L versus mg/dL
Laboratories report Lp(a) in one of two units, and they are not interchangeable:
- nmol/L (nanomoles per litre) — counts the number of Lp(a) particles, and is the unit recommended by modern guidelines. Most UK laboratories now use it.
- mg/dL (milligrams per decilitre) — measures the mass of Lp(a), used by some older assays.
There is no exact conversion because particle sizes vary between individuals, but as a rough guide 1 mg/dL corresponds to approximately 2 to 2.5 nmol/L. Always check which unit appears on your report before comparing your number with anything you read online.
Interpreting your result
Using the thresholds adopted by the European Atherosclerosis Society and reflected in UK practice (HEART UK):

In mg/dL terms, a level below about 30 mg/dL is generally considered desirable, 30–50 mg/dL borderline, and above 50 mg/dL raised.
Two important points of perspective:
- Lp(a) risk is continuous, not a cliff-edge. Risk rises gradually with the level; the thresholds above are practical bands for decision-making, not biological switches. A level of 130 nmol/L is not dramatically different from one of 120 nmol/L.
- Lp(a) is one ingredient in your overall risk, not the whole recipe. A raised Lp(a) matters far more in someone with high LDL cholesterol, high blood pressure, diabetes or a smoking history than in someone with an otherwise pristine risk profile. This is precisely why the result should be interpreted by a clinician in the context of a full assessment rather than in isolation.
How common is a raised level?
Approximately 20% of the population has an Lp(a) above 125 nmol/L, and around one in ten has a level above 200 nmol/L. If your level is raised, you are in plentiful company — and identifying it gives you an advantage most people do not have: the ability to manage the rest of your risk profile deliberately and early.
Why Does Lipoprotein(a) Matter?
Large genetic and population studies have established Lp(a) as an independent, causal risk factor for cardiovascular disease. In practical terms, a persistently high level is associated with an increased lifetime risk of:
- Coronary artery disease and heart attack — through accelerated atherosclerosis of the coronary arteries
- Stroke — through the same arterial disease processes affecting vessels supplying the brain
- Aortic valve stenosis — Lp(a) is the strongest known inherited risk factor for calcific narrowing of the aortic valve
- Peripheral arterial disease — narrowing of arteries in the legs
The word independent matters: Lp(a) adds risk on top of whatever your cholesterol, blood pressure and lifestyle already contribute. It also helps explain a pattern cardiologists see regularly — the patient who has a heart attack in their forties or fifties despite normal cholesterol, a good diet and no obvious risk factors. Unmeasured Lp(a) is one of the more common missing pieces in that puzzle, alongside family history.
Standard cholesterol tests do not include Lp(a), and it is not part of the routine NHS Health Check. It must be requested specifically — one reason so many raised levels go undetected.
Who Should Have Their Lipoprotein(a) Measured?
Because levels are stable across life, a single accurate measurement is usually sufficient — one test, once, gives information that remains relevant for decades. Current European guidance suggests every adult should have Lp(a) measured at least once. Testing is particularly important if any of the following apply:
- A family history of early cardiovascular disease — a parent or sibling with a heart attack or stroke before 55 (men) or 65 (women)
- A relative with a known raised Lp(a) — cascade testing of first-degree relatives is recommended
- A personal history of heart attack or stroke at a young age, or arterial disease that seems out of proportion to your conventional risk factors
- Familial hypercholesterolaemia or high cholesterol that responds less to treatment than expected
- Aortic valve stenosis, particularly if diagnosed at a younger age
- An intermediate cardiovascular risk score, where the result may refine decisions about preventive treatment

Testing involves a simple blood test; fasting is not required for Lp(a) itself.
My Lp(a) Is Raised — What Can Be Done?
This is where accurate, up-to-date information matters most, because the landscape is changing quickly.
1. Reframe the goal: manage total risk, not just the number
At present, the cornerstone of managing a raised Lp(a) is aggressively optimising every other cardiovascular risk factor, because overall risk is what determines outcomes. In practice this means:
- Lowering LDL cholesterol decisively — statins remain first-line for overall risk reduction. It is worth knowing that statins do not lower Lp(a) (levels may even rise very slightly), but they substantially reduce heart attack and stroke risk in people with raised Lp(a), which is the outcome that matters. 2. Medications with an effect on Lp(a)
Some existing treatments lower Lp(a) modestly. PCSK9 inhibitors — injectable cholesterol-lowering medicines used in selected higher-risk patients — reduce Lp(a) by roughly 20–25% alongside powerful LDL reduction. Lipoprotein apheresis, a dialysis-like filtering treatment, can lower levels substantially but is reserved for rare, very high-risk cases in specialist centres.
3. Treatments on the horizon
Several drugs designed specifically to lower Lp(a) — by 80% or more — are in late-stage clinical trials, with outcome results expected over the next few years. If these trials confirm that lowering Lp(a) reduces heart attacks and strokes, management will change significantly. 4. Consider imaging to personalise risk
In selected people with raised Lp(a), tests such as a CT coronary angiogram or coronary calcium scoring can show whether silent arterial disease is already present — information that can sharpen decisions about how intensively to treat. 5. Test your family
Because a raised Lp(a) is inherited, first-degree relatives — parents, siblings, adult children — should usually be offered testing. Finding a raised level in a family member decades before any symptoms is one of the clearest wins in preventive cardiology.
Frequently Asked Questions
Do I need to repeat my lipoprotein(a) test every year?
Usually not. Because Lp(a) is genetically determined and stable through adult life, a single reliable measurement is generally sufficient. A repeat may occasionally be advised — for example, if the original test used an older assay or different units, around significant hormonal changes such as the menopause, or in kidney and liver conditions that can shift levels. Your clinician can advise on how often monitoring is appropriate in your case.
Can I lower my lipoprotein(a) with diet and exercise?
Meaningfully, no — lifestyle changes have little direct effect on Lp(a) because the level is set by your genes. However, this absolutely does not make lifestyle irrelevant: a healthy diet, regular exercise, a healthy weight and not smoking substantially lower your overall cardiovascular risk, which is the outcome that matters. Think of it as compensating for the factor you cannot change by optimising all the ones you can.
Is a high Lp(a) the same as high cholesterol?
No. Standard cholesterol tests measure LDL, HDL and triglycerides — they do not include Lp(a), which is a distinct particle with its own genetics and its own contribution to risk. You can have completely normal cholesterol and a high Lp(a), or vice versa. This is why guidelines recommend measuring Lp(a) specifically, at least once.
Should my children be tested for lipoprotein(a)?
Because raised Lp(a) is inherited, cascade testing of first-degree relatives is generally recommended when a high level is found — typically in adulthood, when the result would influence decisions about prevention. Testing in childhood is usually reserved for specific situations, such as a family history of very early cardiovascular disease, and is best discussed with a clinician.
My Lp(a) is high but my doctor says there is no treatment — is that right?
It is more accurate to say there is currently no widely available medicine specifically licensed to lower Lp(a). There is, however, an effective management strategy: intensive control of every other risk factor, which measurably reduces the risk that matters. Selected patients may also benefit from PCSK9 inhibitors, and dedicated Lp(a)-lowering therapies are in late-stage trials. A raised level is a call to action, not a dead end.
What level of lipoprotein(a) is dangerous?
Risk rises continuously with the level rather than switching on at a single number. Levels above 125 nmol/L (roughly 50 mg/dL) are associated with clearly increased risk, above 200 nmol/L the contribution is significant, and very high levels above 430 nmol/L carry risk comparable to familial hypercholesterolaemia. What any given level means for you depends on your overall risk profile, which is why results deserve a proper clinical interpretation rather than a traffic-light judgement.
